Company Accident Report Form

You can fill out the Company Accident Report Form on the MiSVb portal.

  1. Under Company, select Accident at Work
  2. Click Start
  3. Step 1: Accident at Work
    1. Fill out the accident information
    2. Add a witness if applicable
    3. Click Continue
  4. Step 2: Fill out the employee’s information
    1. You can search by last name or ID number
    2. Click the green check mark to fill out the employee’s profession
    3. Click Continue
  5. Step 3: You can upload a document related to the accident (not mandatory).
  6. Step 4: Review the information and click Submit Request.
